Sudden memory of back a few years ago: the X-ray department handed my son their giant envelope to take back to the pediatrician, with the latest enclosed. (This is before they went to electronic records.) There was a large form on the outside with a line for each x-ray taken on this patient--and they'd had to go to a second page by then.
